This math and me "me math" booklet, is a wonderful end of the year activity, where students can put into practice all sorts of math concepts that they've learned.  It also works well as an icebreaker for the beginning of the year, to help everyone get to know each other.

Design your own booklet to fit your grade level, by choosing from 28 pages involving all sorts of math concepts. To conserve paper, the masters have 2 pages on each template.

Do one or two pages each day, or send the booklet home for students to work on with their parents, as a home-school connection.
